Home Health Care Referral Charges

1. Interview and Initial Setup Fee
   A one-time fee of will be charged for case management, setup, and implementation.

2. Referral Fees for Private Duty Caregivers
   A referral fee will be charged to private duty caregivers.

3. Care Plan Notebook
   All paperwork and materials used to set up the patient’s Care Plan notebook are the property of Caring Hands Maui.
